Easiest Method - Please fill out the form provided (you can either fill it out online and click send or print it out and mail it to the address given below.
If you do not have internet access then follow these simple steps:
Write us a letter Advising us when you have been accepted for training, or have entered an accredited university, college, or trade school. The Scholarship funds awarded to you will become available at that time. Or, have the school or organization advise us of these details, and we will handle the matter directly with them. Either way, we must have their written verification.
If you receive itemized billings from the school or organization, please send the bills promptly to s so we can pay the school or organization (remember, we will not reimburse for any monies paid). If your school or organization bills us directly (itemized expenses), we will promptly pay them. In either case, we will only pay the school or organization.
All requests for Scholarship funds must be in writing, we will need the school name and address, contact person information along with telephone numbers, amount of scholarship monies being requested, plus copies of bills or invoices from the school or organization, please send them to Steve Lavoie, PO Box 414 Valrico Fl 33595. Please allow 30 days from the date that we receive your request for Scholarship funds.
The Scholarship Committee shall have the final decision on determining the appropriate use of funds for Scholarship recipients for all matters not covered otherwise in this policy.
Remember, all Scholarship funds will only be paid to the School or Organization, and no reimbursement of paid expenses will be allowed.
